Autofax Information and Form

If a client does not want to receive hard copies of their reports delivered by either a courier or Canada Post, they may request to have their reports auto faxed. To initiate having results auto faxed please complete the Autofax Transmission Request Form and fax your copy to Client Services Department at 519-258-9505 or call 519-258-1991 ext 0 for assistance. Once autofax commences, the client will receive both hard copies and faxes for comparison for the first two weeks. At the end of the 2 week trial period, the “Autofax sign off” letter is faxed to the requesting client for signature and hard copies will be shut off.

 

Currently we do not offer autofax for Histology and Cytology reports.

Flow Cytometry Testing

  • Flow cytometry testing can be performed Monday, Tuesday, and Wednesday at 1428 Ouellette Ave before 12:00 pm.
  • Patients can walk in without an appointment.
  • The test requires a laboratory requisition along with the required Flow Cytometry “Life Labs” form.
  • Patients can either bring a copy of the forms or have them faxed to MLW at 519-258-9505.

PHYSICIAN’S  FREQUENTLY ASKED QUESTIONS

MOH Requisition Requirements

A legible and thoroughly completed requisition will assist MLW in providing the correct tests and results in a timely manner. In order to avoid delays and errors in your patient’s laboratory testing please ensure the requisitions are properly completed.

 

Doctor Information

  • Full doctor name / address / Clinician addess
  • Contact number for urgent results
  • Doctor signature at bottom left – mandated by MOH
  • Date issued at bottom left – mandated by MOH

 

Expiry

  • Requisitions are valid for 6 months from date issued
  • Standings orders for patient testing can only be issued for a 6 month interval, upon ending the patient will require a new requisition to resume testing

 

Copy to Physician

  • Please provide the full name of the practitioner to receive a report. If the report is to be faxed please provide the fax number as well as the name of the person whom the report is to be sent

 

Patient Information

  • Full legal name as it appears on the patients’ health card
  • Gender
  • Date of Birth
  • Provincial Health Card number / version code
  • Patients address including phone number

 

Test(s) Ordered

  • Check the preferred tests or (legibly) print the test name in the “other tests” section

Standing Orders

A physician can request a “standing order” of tests on an OHIP requisition for their patients.

 

A “standing order” requests indicate the time interval and frequency of testing that the doctor has ordered for their patient without having to obtain a new requisition each time for testing.

 

  • A fully completed and legible requisition as mandated by the MOH is required
  • Testing can be daily, weekly or monthly etc. – the duration and frequency of testing must be clearly written on a OHIP requisition
  • The duration of the order is not to exceed 6 months
  • The patient will be informed at their last visit that their standing order has expired and they must submit a new requisition to resume testing
  • “PRN” or as needed is NOT permitted

Flow Cytometry Testing

Testing can be performed Monday, Tuesday and Wednesday at our MLW 1428 Ouellette Avenue location before 12p.m.

OLIS

Medical Laboratories of Windsor is proud to announce that we have officially completed the connection of our Pathology Laboratory test results to the Ontario Laboratory Information System (OLIS).

 

This is in addition to MLW’s Chemistry, Hematology and Microbiology results already being sent to OLIS.  MLW will continue to deploy results to physicians via EMA, fax and paper.  The great advantage of OLIS is that it will offer health care providers a secure, electronic, single point of access to all of a patient’s test results, regardless of which laboratory generated those results.

Patient Portal

Patients can access their test results through the MLW Patient Portal, except for microbiology and pathology results.

 

This service is available for patients with OHIP coverage who are 18 years or older. For more information on how to sign up for the MLW Patient Result Portal, please visit our website and watch the instructional video for detailed guidance

Turnaround time for blood test results

Test results are usually completed at MLW within approximately 24-72 hours. Some tests may be referred to a reference lab, which could result in a longer turnaround time.
